This form should be filled out prior to participating in activities sponsored by the Grace Youth Ministry. It's purpose is twofold: First to allow GYM representatives permission to provide emergency medical treatment if necessary, and secondly to release the Grace Youth Ministry, chaperones or representatives from any associated liability.
Rest assured that Grace Youth Ministry regards the safety of our youth to be extremely important and strives to prevent injuries before they occur, but sometimes accidents happen.
